Accident Investigation, Prevention and Reporting Training provides an overview of accident/incident investigation at various levels and forms training for those who are required to carry out or monitor incident/accident investigations. It is focused on the base understanding of Root Cause Analysis and the tools required to conduct an Incident/Accident Investigation. When incidents are investigated, the emphasis should be concentrated on finding the root cause of the incident so you can prevent the event from happening again. The purpose is to find facts that can lead to corrective actions. 

The aim of the course is to provide each individual with the understanding and confidence required to successfully participate in a Root Cause investigation and producing a sound and comprehensive investigation report. Reacting quickly to the incident with a prescribed procedure and actions can demonstrate your company’s commitment to safety and ensure the proper information is collected to fulfil an incident investigation’s ultimate purpose – to prevent future incidents. 


At the end of this training, participants will be able to: 

  • Know what your responsibilities are as a member of an incident investigation team. 
  • Understand and be able to utilize key incident investigation terminology correctly. 
  • Understand what information is to be gathered during the investigation, including preservation of the incident scene and collection of evidence. 
  • Conduct effective and focused incident investigation interviews without intimidating or placing interviewees in undo duress.

  • Develop appropriate conclusions regarding cause(s) and write effective recommendations for corrective action and report at the conclusion of an incident investigation.
